Decision:



To commission a centralised North Yorkshire Stop Smoking Service that meets the needs of our population in line with evidenced based practice and engagement and consultation feedback.

The key proposals in the new commissioning model to highlight are:

? that the Local Authority moves to commissioning a county wide service overseen by a single organisation responsible for co-ordinating delivery across all service levels and a range of settings. This would mean that the Council would no longer contract directly with individual GPs and Pharmacies (although they would continue to have a key role in the system and the new Provider could enter sub-contracting arrangements to ensure reach and access of service).
? The service will prioritise the targeting of its service at key vulnerable groups e.g. pregnant smokers, people with diagnosed mental health conditions and Long Term Conditions
? The new contract would be paid via a set block payment amount complimented by an incentive payment scheme.

Investment will come from the Public Health Grant allocation, approximate value per annum is £1 million (final budget value TBC). It is anticipated that the contract for the new service will be for 3 years initially, with the option to extend for a further 2 years pending review.

From the 1st April 2013 North Yorkshire County Council assumed key responsibilities for public health.
On transfer of existing contracts from the NHS into the Local Authority, waivers to the Contract Procedure Rules were prepared for permission to continue with service provision, as the timescale for the transfer of contracts was insufficient for the Council to carry out procurement processes to award new contracts.
The contract for the specialist stop smoking service was transferred as a joint arrangement between North Yorkshire County Council (NYCC) and City of York Council (CYC). A NYCC waiver to extend the current contract with HDFT until 31 December 2015 was agreed and signed off by HAS senior management. CYC have obtained a separate waiver for the same timeframe. The intention is to procure separately from CYC.
European procurement regulations dictate that many public services are regularly put out to competitive tender. This is an obligation under European legislation and Stop Smoking services that are currently commissioned by North Yorkshire County Council are now due to undergo this process.
The Public Health Team have been leading the development of a new commissioning model for the delivery of future stop smoking services across North Yorkshire.
